As fee for service practitioners, our policies protect our time and livelihood. We value your time as well as our own.
Intake Forms
Please fill out the Health History Intake Form included in your appointment confirmation email at least 12 hours in advance of your session. If the intake form is incomplete when you arrive, you will be asked to complete it in my office prior to treatment.
Cancellation/Rescheduling Policy
24 hours notice is required to make any changes to your appointment. As a courtesy, you will get a confirmation email 48 hours prior to your scheduled time. All changes, cancellations or rescheduling within 24 hours of your appointment or simply not showing up, will result in a charge of 100% of the treatment cost. Future appointments cannot be scheduled until the outstanding balance is paid.
Appointment Times
The appointment you have reserved is the length of time you have with me and begins as soon as we enter the treatment room. This includes reviewing your health history, assessment, treatment, self-care recommendations and administration. Out of respect and consideration of my time and other patients, please plan to be on time. If you arrive late, I can only give you whatever time remains of your appointment and you will be responsible to pay the full cost of the treatment session.

Privacy Policy
As a Regulated Health Professional, I adhere to the College of Massage Therapists of Ontario's Standards of Care regarding Privacy and Confidentiality. If you would like further information or have concerns, please contact me so we can discuss it. If you are unsatisfied with the resolution, you have the right to make a formal complaint to the Privacy Commissioner of Ontario within one year.
